Baylis Medical Company Inc.

I hereby grant permission to Baylis Medical Company Inc. (“Baylis”) and its representatives to photograph and video me, and otherwise capture my image, and to make recordings of my voice. I further grant to Baylis and its representatives the right to reproduce, use, exhibit, display, broadcast, and distribute these images and recordings in any media, information source, etc. now known or later developed for any purpose as Baylis sees fit, including, but not limited to: promoting, publicizing, or explaining Baylis and its activities, and for administrative, educational, or research purposes. Photographs, video images, and voice recordings are the property of Baylis.

I also understand that Baylis Medical Company Inc. (“Baylis”) and its representatives will use and disclose my personal information with industry partners in order to provide the course. I hereby grant Baylis and its representatives the right to disclose such information to industry partners for the purpose of providing the course.
